Module 14 Case Study on Jerry Jenkins

 

Jerry Jenkins, an 85 year old African American person with age related maculopathy, is interested in learning how to be independent while completing activities of daily living (i.e., cooking, cleaning, and paying bills). Jerry's wife of 64 years died 4 months ago.  Their relationship was very traditional, therefore, he has not ever "kept house".  His visual acuity is 20/400 OU.  His rehabilitation counselor scheduled a low vision examination 1 month ago.  He went to the clinic and received a telescope, a handheld magnifier, and non-optical devices such as a reading stand, contrast enhancers, and a flexible arm lamp.  Using the Feinbloom Distance Test Card, his visual acuity with the telescope was 20/30.

Using the Vision Rehabilitation Plan Form write a vision rehabilitation plan (currently referred to as an individual employment plan) for Jerry's functional assessment and instruction in the use of his vision unaided and with his devices for the activities of daily living he needs to learn how to complete independently to remain in his home.
